New and Continuing Diabetes Education for Clinicians
Preventing, diagnosing and managing diabetes are growing concerns as the number of people affected by diabetes continues to rise. Primary care clinicians are continually challenged with staying up-to-date on new oral medications and other advancements needed to provide optimal diabetes care. This program aims to support our clinicians in applying current best practices and utilizing internal resources effectively, ultimately enhancing the quality of diabetes care across our system. This comprehensive, internal diabetes training program presented by International Diabetes Center, will provide a solid foundation in topics and case studies related to management, complications, treatment options and technologies. Whether you are new to HealthPartners or desire an overall refresher, this program will provide you with a comprehensive base of knowledge in diabetes.
Learning Objectives
Following this learning activity, participants will be able to:
- Identify the referral process to diabetes education and available resources in the HealthPartners system to help you build your diabetes team
- Identify when continuous glucose monitoring would be beneficial for individuals with diabetes and how to systematically view and use the data to adjust medications and suggest lifestyle behavior changes
- Apply strategies to promote healthy nutrition and lifestyle changes with individuals
- Evaluate noninsulin diabetes therapies and identify principles for selecting, combining, and advancing therapies
- Initiate insulin therapy based on patient assessment and adjust insulin doses based on blood glucose monitoring pattern management or using continuous glucose monitoring data
- Outline the latest management guidelines for lipids, HTN, and kidney disease for people with diabetes
