Clinical Diabetes Management for Primary Care Clinicians

Diabetes is a very common and serious disease. New diabetes therapies and technologies are expanding dramatically and evidence-based guidelines are continually changing, making management more challenging. Keeping current with the latest therapies for the management of diabetes and related comorbidities is difficult and time consuming.

Primary care clinicians are increasingly responsible for diabetes management and education. This requires extensive knowledge of the natural history of diabetes and recommended treatment options, as well as the ability to teach people how to successfully manage their disease. Given the complex and diverse nature of diabetes, education on evidence-based practice guidelines and protocols is vital to a clinician’s ability to effectively treat individuals with diabetes.

This program uses lectures and interactive learning activities based on the latest research and clinically proven therapies. With an emphasis on application, this comprehensive program for medical professionals includes practice-based case studies. This program will give providers increased knowledge, skills and confidence, and provide tools to manage this complex chronic disease.

Target Audience

Primary care physicians and nurse practitioners, physician assistants; medical therapy management pharmacists, certified diabetes educators

Learning Objectives

Following this learning activity, participants will be able to:

  • Discuss the underlying defects and natural history of diabetes
  • Evaluate noninsulin diabetes therapies and identify principles for selecting, combining and advancing therapies
  • Initiate insulin therapy based on individual assessment and adjust insulin doses based on blood glucose monitoring pattern management or continuous glucose monitoring (CGM) data
  • Apply strategies to improve adherence to diabetes self-management activities such as lifestyle changes and glucose monitoring
  • Outline a systematic approach for screening and managing the complications of diabetes: hypertension, dyslipidemia, renal disease, neuropathy, wound care, diabetes-related eye diseases, and sexual function
  • Describe diabetes management in special populations such as pregnancy and the elderly
  • Discuss evidence-based strategies to address obesity

Directions and parking

Take Highway 100 to Excelsior Blvd.; east to Park Nicollet Blvd. (behind McDonalds; across from Miracle Mile Shopping Center); turn left into the clinic campus and follow signs to the free patient and visitor parking ramp or valet. Handicapped parking is available on each level of the ramp and noted surface lots near building entrances. Please park on the upper levels of the ramp to allow priority parking for our patients. The campus consists of three clinic buildings – 3800, 3850 and 3900. Enclosed skyways located on the second level of the parking ramp connect the buildings. 
Follow signs to the 3800 building and elevators. Naegele Auditorium is located on the 7th floor.

Additional parking: If you don’t mind a short walk, there is free parking along Wolfe Park as well as a free ramp at Excelsior & Grand, they are about a 4 minute walk to the 3800 building. A local map can be found here. Check with your hotel to see if they provide free shuttle to our location, several do. Parking on-site can be very limited.
