This 3-month MOC improvement project is intended for our internal Primary Care clinicians.

Learning Objectives

Following this improvement project, participants will: 

  • Define both the current obesity diagnosis and coding standards based upon BMI
  • Analyze obesity diagnosis and coding data to identify gaps that exist within personal practice
  • Analyze obesity management program engagement data to identify gaps that exist within personal practice
  • Adopt strategies, tools and resources to improve problem list diagnosis or coding of patients with obesity
  • Adopt strategies, tools and resources to increase engagement in any of the following for obesity management: lifestyle change programs, nutritional services or behavioral counseling
  • Report an increase in obesity diagnosis or coding rates
  • Report an increase in engagement in any of the following obesity management resources: lifestyle change programs, nutritional services or behavioral counseling

Steps to Earn and Claim CME Credit:

  1. Reflect on your personal practice and practice data identifying barriers to improvement. 
  2. Rate your pre-project confidence in holding obesity management conversations with your patients.
  3. View the following: 
    • Project Overview: Obesity Management
    • Obesity Management educational series
  4. Participate in the Unconscious Bias course if not done so previously. 
  5. Throughout the project, review patient data and identify and apply strategies to reduce barriers.
  6. Reflect on outcomes to determine whether the intervention strategies resulted in improvement. If no improvement occurred, reflect on potential reasons.
  7. Complete MOC Attestation/PI CME Evaluation in CMEcentral by November 27, 2026
