Learning Objectives
Following this improvement project, participants will:
- Define the diabetes and vascular measures
- Analyze diabetes and vascular measures data to identify improvement opportunities
- Adopt strategies, tools and resources to support improved care and outcomes
- Report an increase in diabetes and vascular rates
Steps to Earn and Claim CME Credit:
- Watch Improving Diabetes and Vascular Care and Management recording.
- Reflect on your current practice and practice data identifying barriers to improvement.
- Throughout the project, review patient data and identify and apply strategies to reduce barriers.
- Reflect on your project results.
- Complete the MOC - CME evaluation, attestation and credit verification in CMEcentral by November 24, 2023.
