3.    Identify and apply strategies and barriers to improvement - Record reflections monthly.
4.    Complete the project attestation and evaluation and credit verification via CMEcentral. 
5.    Send your Action Planning Worksheet to PortfolioProgram@HealthPartners.com  
Note: CME credit will not be awarded until your worksheet has been received.

Clinicians who belong to one or more of the 18 Medical Boards participating in the ABMS Portfolio Program or NCCPA are eligible to claim Continuing Certification (MOC) credit.
