Improved Integration of Reach Out and Read in Primary Care - 2026

Project Overview

Reach Out and Read is a 3-month, self-directed project designed for physicians and physician assistants to meet continuing certification requirements while focusing on improved literary guidance and book distribution.
 

Aim

Increase the Reach Out and Read (ROR) book distribution rate during well child visits for children ages 6 months to 5 years from 85.65% to 90.4% and review race and pay gap data recognizing and addressing any disparities that might be present.
 

Individual Clinician Measure 

Percent of books given. Efforts to increase distribution from the beginning of the project toward goal of 90.4%. 
 

Steps to earn MOC credit:

  1. Review the Improved integration of Reach Out and Read in Primary Care Project Overview 
  2. Check current book distribution rate in Power BI including race and payor rates. 
  3. Watch, if not already done so, the required MN or WI Reach Out and Read training video
  4. Review the AAP policy statement, Literacy Promotion: An Essential Component of Primary Care Pediatric Practice: Policy Statement | Pediatrics | American Academy of Pediatrics
  5. Reflect on opportunities to integrate books/reading into your anticipatory guidance with parents.
  6. During well-child visits, provide developmentally tailored literacy counseling and ensure consistent distribution and documentation of appropriate books for a minimum of 3 months. 
  7. Reflect on outcomes, noting whether the interventions improved distribution rates and identifying barriers if not. 
  8. Complete the MOC Attestation in CMEcentral by November 27, 2026.


Participation Attestation

To receive Continuing Certification (MOC) credit, clinicians must attest to participation and meeting all project requirements.

This improvement project meets the following medical boards’ physician MOC Part IV requirements and NCCPA’s CME credit:

  • Family Medicine (ABFM): Performance Improvement Activity (20 Points)
  • Internal Medicine (ABIM): Practice Assessment (30 Points)
  • Pediatrics (ABP): MOC Part IV (25 Points)
  • NCCPA: 30 PI - CME Credits
