This 4-month MOC improvement project is intended for our internal Primary Care clinicians.

Learning Objectives

Following this improvement project, participants will: 

  • Define organizational measurement goals for childhood and adolescent immunizations
  • Analyze personal immunization data including the addition of health equity data on race and payor gaps and identify opportunities for improvement
  • Adopt strategies, tools and resources to support improved care and outcomes. (Overdue patient lists, HMA prompts, Best practice checklists, Quick Schedule, etc)
  • Report an increase in childhood and adolescent immunization rates

Steps to Earn and Claim CME Credit: 

  1. Watch Improving Childhood and Adolescent Immunization Rates WebEx.
  2. Reflect on your current practice and practice data identifying barriers to improvement.
  3. Throughout the project, review patient data and identify and apply strategies to reduce barriers.
  4. Reflect on your project results.
  5. Complete the MOC - CME evaluation, attestation and credit verification in CMEcentral by November 24, 2023.
