Coding and Documentation Integrity Ambulatory - Live Training
This session provides physicians and APCs with practical guidance on how to ensure that their clinical documentation accurately reflects the complexity and scope of patient care. Participants will review current E/M coding guidelines, learn how documentation elements support appropriate level selection, identify common areas where incomplete or ambiguous notes can lead to under or over-coding, and learn the importance of documentation specificity and accuracy for appropriate risk adjustment. Specialty-specific examples will illustrate how clear, concise documentation supports both compliant and full recognition of clinical effort while ensuring strong continuity of patient care.
