Improving Childhood and Adolescent Immunization Rates
Learning Objectives
Following this improvement project, participants will:
- Assess the importance of immunizing children and adolescents
- Define organizational measurement goals for childhood and adolescent immunizations
- Analyze personal immunization data and identify opportunities for improvement
- Adopt strategies, tools, and resources to support improved care and outcomes. (Overdue patient lists, HMA prompts, Best practice checklists, Quick Schedule, etc)
- Assess for improvement in immunization rates/measures during the intervention period (Baseline, Midpoint, and Endpoint)
Steps to Earn and Claim CME Credit:
- Watch Immunizations MOC Recording and Prep
- Reflect on your current practice and metrics - Record on your Action Planning Worksheet.
- Review your practice data monthly.
- Identify and apply strategies and barriers to improvement - Record reflections monthly.
- Complete the project attestation and evaluation and credit verification via CMEcentral.
CME credit will not be available until your worksheet has been submitted.
To guarantee credit in 2022 submit your completed Action Plan Worksheet and complete the Evaluation, Reflection/Attestation by Nov 18, 2022.
