- Apply strategies to implement workflows and utilize tools that enhance care and support for patients requesting and using CGM in primary care, through effective team-based coordination, documentation, referrals and patient education
- Identify in Epic how to access, complete and print a completed Asthma Management Plan SmartForm
- Define the process for patients graduating from specialty to primary care and know where to find specialty communication about assessment and plan
